If you require any vaccinations relating to foreign travel you need to complete a travel questionnaire. This can be downloaded or submitted electronically to us but please remember that you will need to contact the surgery about 7-10 days after you have sent the form to enquire what vaccinations you require. The Nurses will have assessed your vaccination requirements based on the information provided so please give as much detail as possible. Please note the surgery will not ring you if you need vaccinations, you must contact the practice to check your requirements.

It is important to make this initial appointment as early as possible - at least 2 months before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

A private prescription will be provided for malarial treatment.
Payment for vaccination courses is required prior to your first appointment as some vaccinations have to be ordered in. You need to pay in advance because sometimes patients change their mind and when this happens the practice incurs a cost for the vaccines and may not be able to use them for other patients if the shelf life is too short. Cheques should be made payable to Green Street Clinic.
Please remember it is the patients’ responsibility to contact the surgery to make an appointment with the practice nurse after completion of the Holiday Vaccination form.
Whilst we will always try to accommodate patients who have booked holidays at short notice, at busy times this may not be possible.
